Our Hospice of South Central Indiana is Hiring a Hospice LPN Near North Vernon, IN

Our Hospice is looking for a Licensed Practical Nurse (LPN) who will provide in-home hospice nursing care needs of an assigned group of patients/families under the supervision of a Registered Nurse Case Manager (RNCM) within our Columbus, IN service area. Professional nursing services will be in accordance with physician’s orders and plan of care established by the Hospice team. This position supports the RNCM directives while implementing Home Health duties based on need.

This role will be Monday-Friday, no holidays or weekends. The hours are 8 am - 4:30 pm.

An LPN at Our Hospice Will:

  • Provide routine hospice nursing care to an assigned group of patients in a cost-effective, professional and quality manner.
  • Support development of the plan of care for patient/family by collecting and sharing data/patient information with RNCM.
  • Educate and counsel patient/family concerning pain and symptom control, dietary instruction, and physical care needs.

An LPN at Our Hospice Needs:

  • Active Licensed Practical Nursing (LPN) license in Indiana
  • One year experience in health care preferred
  • Valid Indiana Driver's License
  • Interest and Ability in providing patient care in homes and facilities
  • Reliable vehicle for home visits
  • CPR

Salary Range: $22/hr - $25/hr plus mileage

Our Hospice employees are eligible for Public Service Loan Forgiveness (PSLF)
